Job Details:
- PRN - Flexible Hours - 20 - 30 hours per month (you determine schedule)
- NO call!
- Community-based program - build rapport with patients in an intimate setting
- Mileage reimbursement available
- Performing visits in a 2-week time frame to comply with the provided guideline
- Understanding of hospice and palliative environment experience in this environment is preferred.
- Enjoy practicing autonomously with virtual support from a tight-knit team of a collaborating physician and peer APPs
Your Qualifications:
- Graduate of a nationally accredited Nurse Practitioner (NP) Program
- Current NP licensure in Illinois
- Ability to work autonomously
- Experience in palliative care, hospice, or geriatrics required

The posted pay range for this position is what SSM Health reasonably expects, in good faith, to offer based on the circumstances at the time of posting.
